Please sign the waiver using the the link below before entering the facility.  This waiver is specific to the Terasaki Budokan Gym Facility.  You will ONLY need to sign this waiver ONCE on your first visit.  Note: You will NOT receive a confirmation e-mail.   Please take a screen shot of your completed waiver and show the security guard at the entrance.  Otherwise, verbally let them know that you signed it.
